• Description: Provides intake, assessment and evaluation to determine eligibility for individuals with developmental disabilities. Case management coordinates services for the following: nursing, nutrition, medical consultation, respite care for families, parent education, family training, speech therapy, hearing and vision services, physical and occupational therapy, psychological services, assistive technology services, court ordered evaluations, limited conservatorship services, and representative payee services for governmental benefits. Genetic risk assessment and counseling and parental diagnosis are also available to families.

• Eligibility: Applicants must have a developmental disability or similar condition, or be identified as at risk in the case of infants and young children. The disability must have been incurred prior to the age of 18, be likely to continue indefinitely, and is a substantial handicap for the person. The person must be a resident within the FNRC service areas. Anyone may refer a resident of the nine counties served by FNRC for services if the individual is suspected of having a developmental disability or believed to be at high risk of parenting a child with one. Once a referral is received, FNRC staff will meet with the person referred, review existing records, and conduct an assessment of eligibility. Developmental disabilities include intellectual disability, cerebral palsy, epilepsy, autism, and other neurological conditions closely related to intellectual disability or requiring similar treatment.
